Contracting Instructions
You will need the following items to complete the document:
- Social or EIN
- Copy of “resident” insurance license (you will only need the state abbreviations for non-resident states)
- Copy of E&O policy
- ACH Bank Info
- Beneficiary Name for commissions payable after death
- Name of direct up-line / referring agent.
