Contracting Instructions for Humana
Below is the Humana form you will need to complete in order to get started. Return it to the Contracting Department at North American Insurance Services for processing via fax at 469-362-1314 or email to firstname.lastname@example.org. Please keep in mind completing the attached form is not the actual contract, just what begins the contracting process.
Next, you will receive an email with the subject line “Humana Contract Invitation” so you may start your online contracting.
Once the online contracting has been completed, within 3 to 4 business days you will receive an email from Humana with your SANS # and the certification link and instructions. You will not be able to present or sell Humana until all steps are completed.
If you are appointed with Humana through another FMO/MGA, you must obtain a release letter and submit with the attached forms.
IMPORTANT: You must have your AHIP up-to-date in order to be ready to sell!
